Q How long does it take for me to implement SNFQAPI in my facility?

A SNFQAPI comes ready to go and is fully automated with indicators pre-assigned to each of the five committees on a monthly basis. All you need to do is register one time and type the email addresses of your committee members and you are good to go.

INDICATORS

Q What is an Indicator?

A Indicators are online tools used by QAPI committee members to determine if your facility is in compliance with regulations and facility standards that the Administrator or corporate office have established. Indicators measure compliance in clinical care, quality of life, resident choice, management practices and facility standards. Using any computer, smart phone, iPad or wireless device, simply answer each question by clicking YES or NO and see the estimated scope & severity, percentage of compliance and findings change. If you require an immediate plan of correction, click the red blinking letter. In addition, SNFQAPI provides links to CEU providers, educational videos, and vendors advertising products relating to your indicator. When you are finished with your assigned indicators, click the PUBLISH button on the bottom right. In seconds, SNFQAPI emails a completed CMS-2567 to the addresses you have chosen. SNFQAPI comes ready to go and is fully automated with indicators pre-assigned to each of the five committees on a monthly basis. All you need to do is register one time and type the email addresses of your committee members and you are good to go.


Q What do they do?

Q What do I need to complete my assigned Indicators?

A You can use any computer, smart phone, iPad or wireless device.


Q What do I do on my Computer?

A Simply answer each question by clicking YES or NO as it relates to a sample group of six. See the estimated scope & severity, percentage of compliance and findings change. If you require an immediate plan of correction, click the red blinking letter. When you are finished with your assigned indicators, click the PUBLISH button on the bottom right. In seconds, SNFQAPI emails a completed CMS-2567 to the addresses you have chosen.


Q Can I make my own Indictors?

A Yes. Create your own indicators by simply clicking "add new indicator" under the "Dashboard" drop-down menu in the top left of the screen. Some facilities customized indicators include: policies and procedures, litigation avoidance, customer satisfaction, medical records audits, quality measures, readmission tracking, adverse events, etc.


Q What is a SNFQAPI page tutorial?

A On the bottom left corner of most pages you will see a CLOUDS ON OFF button. If you click ON you will see Clouds with instructions explaining how to use that page. If you click OFF, they will go away.


Q What do I type in each of the "SAMPLE" boxes?

A If the Indicator asks to interview residents, families or employees, type the first initial and last name. If the Indicator asks to evaluate beds, you can use the room number, 12A, 24B, etc. You can type anything you want. The SAMPLE box identifies what you are measuring to determine compliance.


Q What happens when I click on IMPORT MDS? (This feature is currently being BETA tested.)

A When you click IMPORT MDS the SAMPLE boxes automatically fill. You don't have to type anything. For example, if the Indicator asks: Is there evidence the resident received the necessary treatment to prevent new pressure sores from developing? SNFQAPI automatically populates the SAMPLE boxes with the names of SIX residents with identified pressure sores. Another example, if the Indicator asks: Is there evidence the resident's clinical condition demonstrated the weight loss was not avoidable? SNFQAPI will automatically populate the SAMPLE boxes with the names of SIX residents with a significant weight loss over the last thirty days. Each time the IMPORT MDS button is pressed a new selection of resident names populate the SAMPLE boxes. SNFQAPI analyzes facility MDS data against national averages.


Q Can I add my own comments to the findings on each Indicator?

A Yes. Within seconds, findings are written by SNFQAPI and state the deficient practice, names of residents affected by the deficient practice and percentage of compliance. You can edit the findings as you see fit or click the CLEAR button and write your own. The findings print on the CMS-2567 form.


Q The Indicator I've been assigned is not applicable to our facility?

A On the Dashboard select the correct Category from the drop down menu and uncheck the box of the Indicator that does not apply.


Q What happens if I don't finish all of my Indicators in one sitting, will I lose my work?

A No. Your information is saved as you type it. SNFQAPI was written by health care professionals for health care professionals. We get it.

Q Does SNFQAPI write a Statement of Deficiencies and Plan of Correction?

A Yes. SNFQAPI has thousands of Plans of Corrections written by State Surveyors, Physicians, Administrators and seasoned Health Care Professionals. CMS estimates it takes an Administrator approximately two hours to complete one CMS-2567, "Statement of Deficiencies and Plan of Correction". SNFQAPI completes the task in five seconds.

Our POC’s include the Ftag, estimated Scope & Severity, Regulation, Statement of Deficiencies, Findings and Plan of Correction that addresses: how corrective action(s) will be accomplished for residents found to have been affected by the deficient practice; how the facility will identify other residents having the potential to be affected by the same deficient practice and what corrective action will be taken; what measures will be put into place or what systemic changes the facility will make to ensure that the deficient practice does not recur; how the facility plans to monitor its performance to make sure that solutions are sustained; how the POC is integrated into the Quality Assurance Performance Improvement system and how the corrective action is evaluated for its effectiveness.

There’s not one POC that can address every issue, so be sure to edit each CMS-2567 to meet your individual needs.

DASHBOARD

Q What is a Dashboard?

A Although SNFQAPI comes preformatted and ready to go, your QAPI Chairperson can customize the Dashboard to meet your facilities specific needs. For example, if you received multiple deficiencies on your last annual survey, mock survey or as a result of a complaint visit during the year, you can set up SNFQAPI to monitor those indicators more frequently. Simply select the committee from the drop-down and choose the indicator you want to be reviewed and click on the months you want committee members to review it. Hold your mouse over any indicator and see the question pop-up.

SNFQAPI identifies deficiencies by analyzing committee member responses and automatically reassigns deficient indicators the following month. Sub-standard deficiencies and potential litigation Ftags can trigger multiple indicators to be monitored. Deficiencies turn red on the dashboard to identify problem areas and trends. Deficient practices are automatically reviewed monthly until the facility is 100% in compliance. Create your own indicators by simply clicking on "add new indicator" located on the bottom right of the page. LIBRARY

Q What is the Library?

A Our digital library is a place where SNFQAPI users share information. It is an informal collection of forms, excel spreadsheets, word documents, powerpoint presentations, literary and artistic materials, books, periodicals, newspapers, CDs, and videos uploaded to the site by health care professionals.


Q Can I upload the forms I use in my facility?

A SNFQAPI respects copyright holders and complies with all copyright laws including the Digital Millennium Copyright Act (DMCA). When we receive proper notice that inclusion of material in our library has infringed on a copyright, we will take immediate action by removing that item from the library. We use Copyscape to scan for copyright infringement. If material submitted appears to infringe on a copyright, we may reject it and refuse to include it. The library is available for user convenience only. SNFQAPI makes no recommendation or guarantee as to the accuracy of any of the information or the content of any material that is found in the library.

BILLING

Q Where do I get a W9 for our business office to set-up SNFQAPI, LLC as a vendor?

A On the bottom of any page click W9 and print.


Q I registered our facility on SNFQAPI using my Credit Card. How does the BILLING work?

A When you first register with SNFQAPI, your Credit Card will be charged for the remaining days in the current month and the regular amount on the first of each month thereafter. On your statement you will see charges from SNFQAPI, LLC. We utilize PayPal with encrypted connections for your security and don’t retain credit card information.


Q How do I cancel my account with SNFQAPI.com?

A To protect facilities from losing all information, user settings, indicators and CMS-2567 history, SNFQAPI requires cancellations to be in writing, signed by an authorized person. Please send your request to: SNFQAPI, LLC, 3696 Crown Hill Drive, Santa Rosa, California 95404. Upon receipt, your facility will be deleted and your billing will stop. When an account is closed, all facility information is deleted from our servers and cannot be recovered.
